Health Information Management

Health Information Management, also known as “Medical Records,” is a vital part of your patient experience. Once you leave our facility, we are honored to continue serving you by providing your information timely and as requested. We are located in the basement of the main hospital at 1401 W. 5th St. and our hours are Monday – Friday 8:00 am to 5:00 pm.

Patient Medical Record FAQs

You can reach us by text or phone at 307.672.1070, by email at HIMRelease@sheridanhospital.org, or by stopping by our office. We kindly ask for 48 business hours to process your request when possible.

Per Wyoming state law, we require a valid Patient Release of Information Form before releasing medical records. If you have not completed one before or yours has expired, please click the link below and complete the form (we require a physical signature, not a typed one). Please return the form via email at HIMRelease@sheridanhospital.org, fax 307.675.6762, mail it to 1401 W. 5th St. ATTN HIM, or drop it off at our office. If you have questions or need assistance with the form, you can reach us by text or phone at 307.672.1070.

We can email or mail records, or you may come to the office during business hours to pick them up. For radiology images, we will email you a link to set up an account to view your images. If we have the pleasure of seeing you in our office, we will ask for your driver’s license or other form of ID to verify your identity. If you are a parent asking for information regarding a minor child, a photo ID of the child is not required- just a photo ID of the parent or legal guardian.

Birth Certificates

Health Information and Records does not keep birth certificates in the medical record. Birth certificates are the property of the State of Wyoming. It is unlawful for us to provide you with any of their forms that are in the medical record, such as the mother’s worksheet or paternity papers. That information must be obtained through the State.

To request your birth certificate, visit Wyoming Vital Statistics Services. On the page’s left side, click “Certificates – Birth, Marriage, and Divorce.” Be sure to send the completed form to the address provided on the form.

NOTICE of Intent to Destroy Medical Records

Healthcare facilities must keep patient medical records as federal and state laws require. Once these requirements are met, the records can be destroyed according to the facility’s policy.

Beginning August 3, 2024, we will begin the process of purging old medical records. After this date, these records will no longer be available. If you need a copy of your medical record, please request it before they are destroyed.
